Remaining issues: write introducition and conclusion (including more urgently the conclusion to the first half). Fix various consistency issues (decide whether the authorial voice is "I" or "we", whether the identity matrix is I vs. \one vs {\bf I}, \cL(H) or End(H) to denote matrices, whether there's a 1/2 in front of trace distance, blah blah blah). Fix some of the figures (\Usch for exmaple) and maybe add a few (like a teleportation circuit for the S_n CG, and maybe an example of protocol using unitary gates).
